Good Shepherd's Social Services Department is committed to helping residents attain or maintain the highest practical level of physical, mental and psychosocial well-being.  Two licensed social workers are on staff to help residents transition into our nursing home as well as discharging out of our nursing into the outside community.

Good Shepherd's social workers help families and/or residents complete the necessary admissions paperwork as well as review residents rights, health care privacy act, etc. Good Shepherd's Social Workers are part of our interdisplinary team that completes a comprehensive assessment of each resident and develops an individualized care plan. regularly in care plan conferences with the resident and family.  Good Shepherd's social workers also are involved in discharge planning so that resident can access the community services necessary to live in his/her community.
